Mobile no. SIM IS INACTIVE

I bought AIRTEL sim and have been using it. At the time of buying I gave proof
identity and photo. Recently I started to face a problem that whenever I call to any number, it is coming as "You have active call barrings." When I asked Customer Care they replied that I have to produce proof. Then I contacted AIRTEL office to give proof again. They didn't take and said that it would be activated. Airtel IPTV connection not shifted to my new Residence

I bought IPTV connection on Airtel existing plan 999 PM (Landline + Internet) after paying one time charges INR 3999. When I bought IPTV got assurance from Airtel Sales representative that the entire connect may shifted to another location.

Bill after Surrender of Services

Please note below the complete history of my complaint. Till date there is no response from the Appellate & Nodal authorities, other than automated acknowledgements.
