Axis Bank — debit card personal accidental cover

My accident happen 28/9/2017 then i went in axis bank for debit card personal accidental cover but they refuge me to give me any form and tell that your card not suitable for this, while my card is used frequently before accident and i intimate to bank about accident with in 90 days.
