Axis Bank — pf amount not credited in my account

I am Juri Deka I have applied for my PF amount. My last working day was[protected]. It's was more then 1 month but PF amount not yet credited. Employee ID of Axis Bank 125959 and my UAN No. [protected].
I have attached my PF statement and also the screen shot of online PF withdrawal request.

Hi,

When i send a amount 6000 to my account in Axix Bank.Then in my account balance shows only 5625.

Where is the balance amount. I tried to call them. They are not attending and send a mail to them. No reply still yet..

Why the banks are cheating??
