Bajaj Finance — regarding provident fund amount not yet credited

Dear sir
I am sunitha emp code e88543, employee of bajaj finance ltd from 1 may 2015 to 30 dec 2015 and i have resigned from the job in 1 jan 2016, for some health issues. And again i have joined the bajaj finance on 1 st may 2017 to 30 oct 2017.in the both time period i have not apllied for pf. So now i am applying for pf but the addecco consultancy hr not accepting the pf application form and i dont know the reason for rejection kindly process my pf asap.
