Bombay Hospital — Wrong discharge summary so that patient is unconscious

My father is hospitalized in bombay hospital on 12.10.2016 (room no.1008 in the name of mr. rajesh kumar dave) and discharged on 14.10.2016.in this regard, please note that hospital staff has wrongly given me discharge summary of mr. shishir kumar instead of mr. rajesh dave in which staff of your hospital brief me medicines which has been shown in discharge summary of mr. shishir kumar. after discharge of my father i left the hospital along with discharge summary (along with all documents) and immediately purchase medicines and given to father as described by hospital staff. after taking only one doze of medicines my father is unconscious and my family is in tension what happened they did not know. they immediately call me and i have seen that medicines which were brought by me on the basis of discharge summary is of mr. shishir kumar instead of mr. rajesh kumar dave.

You are requested to please immediately refund me amount of rs.600.66 towards medicines which were purchased by me from aaroyga (government medical stores) and take strict action to those nurses who are responsible for the same under intimation to me.

What happened if my father...

Today also my father position is not well and good becuase of wrong medicines due to those irresponsible hospital staff. if you not believe please contact me i will produce you all the documents which were wrongly given by your hospital staff as well as also see my father position.

I am not asking for penalty but your strict action against the staff who are responsible for the same is well and good so that in future nobody suffer from this type of situation.

If you are not taking immediate action i have no option but to take some more strict actions against the hospital as well as irresponsible staff.

Dear Sir,

Myself Rajesh Kumar Dave like to inform you that I am hospitalized in Bombay Hospital on 12.10.2016 and discharged on 14.10.2016. In this regard, please note that only Rs.11, 589.00 (Rs.Eleveen thousand five hundred eighty nine only) has been billed by Bombay Hospital against which only Rs.7511.00 (Rs. Seven thousand five hundred eleveen only) has been sanctioned. Rs.4078.00 (Rs.Four thousand seventy eight only) has been deducted by Insurance company. The details are as under:

(*) Rs.2100.00 (Rs.Two thousand one hundred only) against Room Rent
Rs.1778.00 (Rs.One thousand seven hundred seventy eight only) towards Surcharge
Rs.200.00 (Rs.Two hundred only) towards Registration charges.

(*) I can't understand why you deduct Rs.2100.00 against room rent. My policy is of Rs.1.00 lac and paid premium of the same and I am not a regular patient to be hospitalized. In this regard, please refer my Insurance Policy No.151401/48/2016/5748 which is enclosed herewith.

Also please note that all the documents related to the above had already been been submitted by the hospital. If you require any documents pertaining to above, please inform me.

You are once again requested to please release me amount of Rs.2100.00 ASAP without fail.

Dr. Manish Jain give me proper reply but from hospital's end no proper reply. They misguide me.
