Respected sir,
I am undersigned sanjay bhagwankar sending this mail to you because i want to file complaints against lic for my (Jeevan saral surrendered policy). I had made several communications with lic but i am not happy with their reply.

Bad Service

I've had LIC's Jeevan Saral Policy for more than a Year now. I pay premium monthly and intially I set up ECS to pay my monthly premiums. In June, I joined a new employer who opened a new salary a/c. I had to cancel the ECS from the old bank a/c and set up a new one.
I filled the relevant form and submitted to the LIC agent who confirmed the ECS will be in force from next month. However, My bank didn't receive any instruction...

I had purchased Jeevan Saral Policy from Mumbai Branch No.907 , wallace street , fort, mumbai -1.
My Premium mode is through ECS facility from my savings a/c of Rs.1000/- which is like a SIP Plan.
Now, I require a Premium certificate statement for my tax declaration purpose from April`2010 till Dec`2010. My Agent is outstation. I called the call-centre informing them of the same but no response...

Non Receipt of Policy Document

Sir, I have deposited an amount of Rs. 12010.00 to LIC of India, branch code No 95L. through Dev. Officer code : 0007952, Agent Code : 1041595L.
