I have got sms from Credit card provider regarding debited the premium amount.
Please confirm the payment and send E-Receipt as early as possible.

The complainant is having LIC policy bearing no.[protected] under Table and term[protected] HLY installment, date of commencement[protected]. It is money back policy with profits + acc. benefit on every 5 years from commencement. As per terms and condition of policy issued by LIC amount of 20% sum would be returned by the LIC in the month June 2007 and June 2012. But nothing has been returned till so far. Here it is necessary to mention that...

I,Simit Modi,had issued a cheque of amount 8925/- on 29.11.2009 for a policy of Amulya Jeevan with risk cover of amount 2500000/- to your authorized agent Mr Alkesh Modi. Cheque is already cleared & medical test formalities are done by me in Gandhinagar. But after some days i was communicated that I have reappear medical in Mehsana.
I have also cleared it.Some days later I was asked to submit I.T. return...
