Citibank — Not able to apply online credit card from Surat location

My self referred credit card to my colleague, but when he click on referral link and apply for the credit card at that time he entered all the personal details but he is not able to enter the pin code of the surat location, both office and residence

please help us to apply from surat location for credit card
