It is to bring to your kind notice that my & my family Voter ID is not yet recieved due to the carelessness of concerned departments even after filling form 6 with all details including Photos, and submit to concerned Lekhpal and BLO of my Local Area twice. I had been submitted the voter ID application form 6 in SEP./ Oct, 2011 twice .Please look into the matter and instruct the concerned to issue me and my family Voter ID Card at earliest.This representation is submitted for your kind necessary early action from your ends please.The details of mine and my family are as follows.
