But if I enquire in PF Office, I receive the information as "Form not yet submitted"
Request you to help me in this regard.

I worked with Visaka Industries Limited at Lucknow Office from 29.08.2005 to 25.05.2006. I left Visaka Industries Limited on 25.05.2006.

My PF Account No. is 1387 in Visaka Industries Limited, Corporate Office in Secunderabad.

I have received information two cheques(Chq NO:129522, Dt:24.04.2007 Amount Rs.9, 711.00 and another Chq NO:576431, Dt:24.04.2007 amount Rs.4, 080.00.
) are bounced from my bank account in ICICI account no. is[protected] because of some mistake in my name wote on the said cheques.

You are kindly requested to send it again with correct name - ANIL YADNESHWAR MEHENDALE to my another account in ICICI bank account no. is[protected] address of bank is Pune Satara Road, Pune.

You are kindly requested to look into the matter and take the necessary action at the earliest.

This is with reference to my provident withdrawal, My company name is M/s Rain Industries Ltd( producers of Priya cement ) in the year Nov 2007, after that the name changed to M/s Rain commodities Ltd, i resigned in the month of Nov 2007, in the month of June 2008 i submit all the relevant documents to the company for PF withdrawal, In the month of Dec 2008 i ask the status of my claim, the company accountant told your documents caught somewhere, again sent one complete set to claim, then in the month of January 2009 i sent the application to M/s Rain commodities Ltd ( producers of Priya cement ), still now i didn't know the status of my claim, if i make a call to the company they told i submited all the relevent documents to Hyderabad Provident Fund office, Please do the needful,
