[Resolved]  ESIC — Not yet received my New ESI ID card

Dear Sir / Madam, This is to inform you that, i have attend the schedule of Photo capturing for New ID card on dated 07.05.2010 . But yet to get my card, several times i have given complaint to the local office but till not received the same . A person of the department has advised me to give an application by mentioning that I have been misplaced my card and please re issue the same, I followed the same process and applied on 12.04.2013 for new card with a fine of rs.50/- .But till not received the Card . Requesting you to please look at the matter and kindly issue me the Card as soon as possible .
