Dear Sir, Ref: My National Insurance Policy No. 500500/48/10/[protected] I am a policy holder of National Insurance Company since 2007. But, till date, I have not received my Vipul Med Corp Card and in this regard I have written a letter to the Manager, National Ins. Co. on 24th June, 2010. I request you to kindly look into this matter and arrange to send me the ID Card immediately. Your early action will be highly appreciated. Ref: My and my wife and my Son are having United India l Insurance Policy ID No. [protected] 26 /[protected] 03/[protected] 04 Name and branch of insurance 201401-UIC, B.O I valid from 01/12/2016.
I am a policy holder o[censored]nited India l Insurance Company 2016-17 . But, till date, I have not received my Vipul Med Corp Card till date.
I request you to kindly look into this matter and arrange to send me the ID Card immediately.
