Kotak Life Insurance — non receipt of policy.

I have filled up required form for Kotak life Insurance and deposited all required proof (as enclsores) and a cheque No. 979161 of Punjab National Bank, nasik for Rs. 40000. The amount was debited from my a/c on[protected]. Till date I have not received my policy. In the mean time your rep. on telephone asked me to attend me for some medical check-up. It is surprised to note how suddenly your co. is asking me for that, as the same requirement was not intimated to me earlier while opening policy. It it is not possible for me to go to Nasik for required medical check-up, as I am staying at Ojhar ( 23 Kms from Nasik), and I dont have the time for that. In view of the above, it is requested to cancel my policy and refund me my money Rs. 40000 (Forty Thousand) immediately, as I am not interested to continue. Your people are not telling further requirement while making policy , which is against all sort of norm and also not justified. Hope your co. will send me money at the earliest to make a goodwill market. Thanks. Madhumita Naha
