I have been complaining about my telephone instrument not functioning properly since last several days and it is almost dead by now.
In response to complaint, the maintenance staff who visited my residence guided me to lodge official written complaint, as after inspection he found the instrument to be faulty.
I shall appreciate your prompt needful action.

If the Nodal Officer does resolve the issue within 10 days than file appeal with APPELLATE AUTHORITY.
