I Nasir Ahmad resident of E-Block, Gali No. 14, House No. 264, Mukund Vihar, Karawal Nagar, Near Punjab National Bank, Delhi-110094 had purchased a Welfare Endowment Certificate under Social welfare Scheme (Endowment Certificate No. E[protected] on 28.4.1983 (photocopy enclosed) and its maturity was on 28.4.1998 which was already expired.

I had furnished the relevant documents to the Branch Manager, the Peerless General Finance & Investment Co. Ltd., Kanpur Branch, Kanpur for encashment of my policy on 21.5.2009 (Copy enclosed). But I have not received any communication from Peerless Branch Office, Kanpur. I talked telephonically to the Branch Office, Kanpur. They suggested me to send all documents to the Headquarters for encashment of policy. Therefore I am forwarding herewith all documents to you

Sir, I had already submitted all documents to Branch Office, Kanpur for encashment of my policy but no settlement is done to till date. Therefore, I request you to kindly look into the matter personally and pass necessary instruction to Branch Office, Kanpur for encashment of Policy. For this act I shall ever be grateful to you.

But This origianal Certificate has been lost now.Policy Holder: Baneswar Tripathy Sum Asured Rs. 11200/-. at present I have Three receipts Rs.122/- & 280/- deposied on[protected] Rs.400/-deposited on[protected].Now I have not get your dischare form cum voucher form.My permanent Address Vill-saharda. I have ten certificates vide nos. E[protected], E[protected], E[protected], E[protected], A[protected] to 656 which are lying since all are matured but yet I have not received any certificate from you but I have some cash receipts against above no. certificates
I have lodged a G.D on 31.03.2010 vide no. 3320 at phool bagan ps. and thereafter from branch office from 223, C.R. Avenue, Kol-6 accepted only for A series certificate documentation for discharge form and received on 21.04.2010 but yet I have not received any reply or any voucher from the said branch.
