I request you to please give me the format for lodging the complaints against the Mr Ramesh Balram Rai (Self) SB NO:0120067 and aplication no:106826 issud deted 07/06/2000 Add.kulup wadi Murya Chwal Borivali(E)Mumbai who has 2 ration cards in same name.(Holding 2 ration cards and he has good influence in rationing office and ration shopkeeper,

Second is same person staying at Ration Card No:0806233 Ragistar No:4961Issud deted;30/09/2005 at Shop No;33 New surya kirti nagar co-op housing socity ltd achole road Nallasopara(E) Dist-Thane 401209 and he has already having one ration card with his name, but one more ration card is holding with his name,

Please provide me the format for two complaints as per given above mentioned details.

Also provide the address for where should I lodge a complaints.
