I want to withdraw pf, but in that online process my date of birth is showing wrong, please tell me what is my date of birth in company data.

I have requested for FORM3A from Concentrix retirals but the team is reluctant to provide me form 3A for my complete tenure
by merging EPS to PF head. The mentioned lady assumed that by providing form 3A for 6months
(sandhyarani.[protected]@concentrix.com) will help solve my problem. I have been following up for 10 days with no response. Can anyone help?
