Election Commission Of India — lost my election id card

Dear sir,
I sinchana n have lost my election identity card bearing the epic number slc4263786, part number 182 of 216, krishnaraja assembly constituency with 3794238 as my voter number. I have been residing in #544, d block of j p nagar mysuru for past 13 years. I hereby request you to kindly issue election identity card and do the needfull. I have lost my notice without my notice so i cannot identify the place and whereabouts of my lost id card
