Employee's Providents Fund Organisation [EPFO] — online transfer claim -pending status

Dear sir,

I have submitted my transfer claim on 11 sept 2016 but till date it was on pending status and now if i open the online epf member portal the submitted document was removed from there.

Several mail was done to the mail id - [protected]@epfindia.gov.in but no reply received till date.

Kindly resolve at earliest and transfer my claim. Below are the details
