Respected sir, I am writing this regarding the claim made for the compensation for No payment for sickness benefit leave period. My Insurance card no.5207778627. Esi claim amount not settled till now. I have submitted all documents in ESIC, S R Nagar, Erragadda, Hyderabad ESI dispensary on 07.04.2017. The register post ESIC S.R. Nagar delivered date 10.04.2017.But not responsibility in dispensary . Dear team i am working with a National Academy of construction-vijayawada A.P. since last 09 years. I already sent my original leaves certificates and other slip to concerned dispensary and ESIC hospital by register post but since they received my bills there is no response from their side for further process so i request to please help me for the issue to resolve it soon... You are requested to scrutinized the matter. I have enclosed the certificates. Please take a prompt action. Kindly arrange the claim amount at the earliest and Kindly help us solve this issue.
