Dear sir, My name is joseph elias. Working at michael job centre, sulur. Coimbatore -641103, tamilnadu. I am a known patient of glaucoma. I have been advised by the doctor of esi main hostel, sringanallur for "latanoprost" on 24.6.201. After submitting the advise to sulur esi dispensary, i have not yet received the medicine. Even for getting reimbursement i am not getting any clear picture from them. As you know in absence of this drop, patient may loose their eye sight. Kindly take the necessary step to provide me "latanoprost 0.05%" at your earliest. (Referral letters, advise of the doctor and patient's membership card is attached herewith. Regards, Joseph elias I.
