[Resolved]  ESIC — Smart card not received

I am informing that I have not yet received my Smart Card against my Insurance no.[protected] (Mahesh Kumar) At first our picture captured on January, 2013.But due to not receive the smart card but still today I have not received any smart card. Please enlighten me in this matter.My company saying going to ESI Branch & ESI Branch say to confirm by your company. Esi branch say that we are sent to your ESI Card to courier your company.
Please solve my problem

I am Manisha Kaushik W/o Mukesh Kumar. My husband working with Vipul Motors 1-A, 19/6, Mile Stone, Mathura Road, Faridabad as a Job Advisor in Body Shop from 06.08.2014, We have our photo completed in ESIC office, 16, Sector, Faridabad as on 09.09.2014 but we have not yet received our smart card . When we were arrive the esic office in 16 sector to receive our smart card then they said we have not your any smart card and you will have to go 8 sector (esic office) and when we arrived 8 sector esic office then they said we have not your any smart card, your card is in 16 sector esic office and now my husband's health is in very critical situation and need his smart card to treatment please solve my problem and courier my smart card at my home address immediately because nobody are available to go esic office to received smart card and if any problem to give/ courier my smart card then please reply to my mail address([protected]@gmail.com).
