Ex-Servicemen Contributory Health Scheme [Echs] — echs online application site can't be reachable

Myself aniruddha shinde son of pradeep shinde (Ex-serviceman). My father met with an accident on 1st may 2018, currently he is admitted in sancheti hospital pune. He requires some medical treatment which is not affordable. So i am trying to submit online application for echs, but somehow i am not able submit application online. So could you please check & let me know is there any alternative to submit the application.

I filled the form but not completely because of non availability of all the information, but by mistake, payment tab was pressed, now it showing failure in massage and I am not able to EDIT/upload further.

kindly guide so that I can submit my renewal of ECHS card in time,
