I WAS ADMITTED AT FORTIS HOSPITAL AND KIDNEY INSTITUTE AT RASHBEHARI KOLKATA ON 14TH FEB 2015 BUT MY CASHLESS CLAIM WAS NOT APPROVED BY THE HEALTH INSURANCE COMPANY. SO I AM NOW SUBMITTING THE CLAIM FOR REIMBURSEMENT OF THE MEDICAL EXPENSES FOR WHICH ONE PART OF THE FORM NEEDS TO BE FILLED BY THE HOSPITAL. I HAVE VISITED THE HOSPITAL BUT NONE OF THEM WAS WILLING TO HELP ME OUT WITH THE FORM. I HAD ALSO SENT AN EMAIL TO THE HOSPITAL ON 03.05.2015 BUT TILL DATE I HAVE NOT RECEIVED ANY REPLY FROM THE HOSPITAL. Was this information helpful? | |||
