The purpose of this letter is to request copies of my medical records as I was treated in your hospital Mohali dated 2/May/2021. Where I was diagnosed with stones in kidney bladder and urinary track. There they took my blood samples and CT SCANS but they never gave me the reports and they miss behaved with me. I request copies of the following health records related to my treatments.
