HDFC Bank Mutual Fund — Wrong SIP fund purchased folio no

Sir,
I have applied for quarterly SIP of Rs.3500 selected fund Mid cap
opportunities - growth fund where as today mobile message received that folio no
10698571/35 under Hdfc top 200 fund 10.544 unit purchased which is wrongly.
Kindly do the needful to rectify it as soon as possible because i have
choose mid cap opportunities growth fund not top 200 fund.
In my form i have clearly mention it then how change it with out my
intimation.
Ples find the attached scanned copy of form which i submitted to siuri Hdfc bsnk branch.

Hope my concern will be taken care and expecting rectification of it.
