ICICI Prudential Life Insurance — I want to cancel this prudential policy

I am vijay praveen i was working in pharmaceutical company i have been using salary account in icici bank after resigning my company i am finding suspicious deduction in my account i called customer care they told that you have activated icici prudential policy i didn't activate actually so i wanna cancel that policy and return my amount to my account i have added screen shot of it kindly do the needful
