Please let me know my insurance premium account balance sheet of policy number[protected] because i was discontenue for payment of premium on domestic problem for the last few years but i want continue to deposit premium payment so wants to know my balance sheet if previous deposit is accountable or not or how

I applied for the icici credit card.It got returned few days ago but I am getting bills with some processing fee and interest etc.
even after many follow up on mail i did not get my credit card details.
how can someone pay the billed amount without the credit card number..
