My name is aniruddha rairikar

This application is in regards to the policy number:[protected], of which i am the beneficiary, which commenced on the date of 28/03/2002 from lic branch dombivli, branch code 91g.

This policy was bought by my father, late. Dileep rairikar, who died on 24/08/2007, proof of which has been duly submitted to and acknowledged by the branch.

This complaint is to inform you that i, the beneficiary of the policy, have reached the age of maturity, 18 years, on the date 28/12/2017, the date of my birth being 28/12/1999, and have yet to receive the payment due 2018.

I have twice sent emails the dombivli branch in regards to the subject, updating them on my age and the due process required to transfer the policy to my name and current address in indore.
