Income Tax Department — About my PAN card

I took my PAN card a few months before through AKSHAYA. But when i received the PAN card, i found my name was wrongly given. Soon a applied for the correction. But after a month, i got the response as i should attest Annexure A. But thats not the problem. The attesting officer should attach their official id card's copy with this form. I asked for many people but they are afraid to give their id proof's copy. They are saying it contains a pin code and they are refusing to give me. So what should i do now?? I need a replay. If you want me to prove my name, i can give any certificates or even i can go to the village office to get a certificate which can prove my name. But this was really disgusting and disappointing.
