The connection was in the name of my mother smt shanta kumari n she expired on 19.9.16. I fill the necessary forms n copy of death certificate. Vicky agencies inform that for change of name i will have to refund the one time permanant advance of rs.568 and rs.50 as transaction charges. I paid rs.618 in cash to them and they issued me sv in my name. But they donot issue any receipt for refund of advance and transaction fee to me. Simply they write on sv the amt rs.618 n initial on sv. Pl look in to the matter and issue me proper receipt for the above amt paid by me. Reply soon. Thanks. S k malhotra (Old con.
