Irda — removal of pan number from irda portal

I have raised complain previously also but no any steps has taken yet by irda officials. Please do the needful as soon as possible.
I was working with icici bank earlier in 2014 and resigned there in 2014, they gave me noc for agent id of life insurance also but my pan is still active in irda portal. Kindly remove it from there. Sending you the complete information below.
