Irda — surrender license and get the noc letter on prior basis

Respected sir,

I arti, having a agent code of sbi life insrurance co. Ltd. My agency code is[protected]. Termination date of my code is 05/04/2019. My pan card no. Is bldpa7382j.

Now i had got job in another field, because of my name showing in irda i am getting trouble in joining the job.

So, i am requesting you to please remove my name & pan no from irda so i can work in another field. Hope you will accept my request and short out as soon as possible. Kindly provide me noc letter on prior basis.
