I had sent 104 shares of ril for deletion of second name as the second person is no more. Its been six months post . I have not received shares or any communication from karvy. Along with these shares I had sent some more shares of rel capital reliance energy all have been sent with document forms but apparently only reliance industries shares has not been received. I have written to karvy and emailed them 100 times but a computer generated statement is all what comes from there no clue no answer about shares sent. I have filed a police complaint in local town against loss of shares anyone doing transaction in those shares would be legally responsible. I feel shareholders of reliance shud switch to better companies as they are not bothered about there small shareholders and pathetic investor services . tall and bold claims at international levels don't suffice the actual nature of this companies working . pathetic way of management . we are also working towards having shares in demat form if we have shares in physical form there must be some problem at your end solving them due to bar Mr mukesh ambani Was this information helpful? We request you to forward us the below mentioned documents for deletion of name of the deceased holder and transmit the share certificates in favour of joint holder(s). 1. Enclosed form duly filled and signed in all respects. 2. An attested copy of the death certificate. 3. Original share certificate(s). If the share certificates are not available, we request you to execute the indemnity bond and affidavit as per the enclosed annexure for issue of duplicate share certificate(s) in respect of the share certificates not available with you along with original First Information Report (FIR). The FIR should mention the GD No (General Diary Number) and Police station seal (seal should contain National Emblem embossed on it). 4. Provide Proof of address documents like Telephone Bill / Electricity Bill / Voter ID Card / Driving License etc if there is any change in address. 5. Self attested copy of PAN Card(s). 6. Cancelled cheque leaf. Please ensure that the signature(s) of joint holder(s) as mentioned in form /covering letter should match with the specimen recorded with our records.
