Dear sir, I have purchased the sip policy of birla sp96 of rs 2000/- from karvy hazratganj lucknow otm was rejected due to signature differ in the mandate form. I have resubmitted the otm form on dated 04-12-17 to mrs sarika sharma and mr saurabh negi and they misplaced it. There behavior was unethical.
