Kotak Mahindra Bank — passbook issue

I have applied the passbook online and visited nearly 5 branches for passbook none of the bank is having the passbook then what the use of having such bank if they can’t fullfill the basic minimum requirement of bank facilities. From 2 months facing the problem the branches i have visited are ameerpet, begumpet, khairatabad, srinagar colony. None of the bank is ready to give the say will provide in 15-30 days and when reaching them after 20days they repeat the same words when ask them loudly they say we can’t do anything as we are not getting the passbook from center. Now i don’t know what is center. Please solve thid issue

I have attach the dispute form for credit card fraud transaction please solve the paricular mentioned transactions and not reflect the transactios in my statment of accounts do the needful
