Dear sir/madam

I am rani hela joined as insurance agent in kotak mahindra life insurance company limited and my agent code belong to okm60323211 effected from 16 october 2015. Now your agency is terminated me due to non fulfillment of minimum business and i am unwilling to continue with kotak life insurance, so kindly cancel my agent code from your company.

Also i am informing you that due to irregularities in your system data that still show's me as terminated rather than submitted my resignation. Please kindly update my record for further process and convenience (Offer letter already generated from other company) but due to delayed response not able to answering them.
