Dear sir,
I am holding a policy "jeevan prakash" bearing no[protected] wef 25/10/12 with annual premium of rs 62535/- through dsop fund. Now i am (Lt col ps bisht) retired from army wef 01 mar 2017 and wish to send my original policy to the following add :

Request do the needful at the earliest to enable me to pay next premium which is due in oct 17.
