Sir/Madam, I am GULAB CHANDRA YADAV s/o RAJA RAM YADAV, address-WARD 13 DHANA PUR NAGAR PANCHAYT HANDIA, ALLAHABAD.started LIC policy in year 2006, policy no.[protected] from LIC agent Mr.GULAB, agent code no.[protected]as per premium receipt). My premium mode is yearly of Rs.3493.00. In year 2016(due date 11/2016) my LIC agent collected yearly premium cash of 11/2016, but not deposited with LIC. After one year when I go to LIC branch to deposite yearly premium of 11/2017, then I know that yearly premium of 11/2016 not deposited. Then I deposite it with late fee Rs.347.70.
