This has reference to my letter dated oct.11, 2019 regarding correction in the death/ birth certificate as mentioned above (Copies enclosed).
It is a matter of regret that despite writing to you almost one month back no correction action has been taken so far. I once again request to kindly take necessary action in correcting my name as pawan and not pawan kumar as wrongly written in the certificate and also date of death as june04, 2019
