My daughter, a. Krithika was born in the best hospital, kodambakkam, chennai on 05.03.1987. The name of the child in the birth certificate was wrongly mentioned as "a kiruthika" instead of a. Krithika.
I had applied for correction of name as a. Krithika in the dept. Of births & deaths on 29.08.2018. I had submitted all supporting documents like marks statement, aadhaar card, ration card along with sworn affidavit duly notarised. Ack no.1864 dt. 29.08.2018 by public health dept.
The man in the counter said that correction will be effected within 20 days. But even after a lapse of 42days, we find that necessary changes have not been effected by the concerned authorities. As it is urgently required by us, please bestow your personal attention in the matter and make necessary corrections. Thanking you, arumugam.
