Actual : dob 26.03.1988
Application status wrongly showing dob in appln no:8322075.
Status shows pending with crr with incorrect dob:17.08.1998 we have submitted application and 10th certificate for dob proof but still incorrect dob on online status
Rectify correct dob is 26.03.1988 in online status.
Health deptt / mcd incorrectly feeding and forwarding application information

Please rectify this and correct the dob :26.03.1988
