Respected Sir,

As per your mail I am forwarding the complaint form for your ready ref. But it is very true if i Have submitted this claim through any agent then it will pass with in time.Till today i was informed that claim in progress. This is status of today i which it shown that in progress.

I can not understand how the claims are pass By DHS MOS in Corporate HR office which are managed medi claims. And up to my knowledge Sir H.N. Reliance hospital Mumbai will never issue manged bills.

I hope you will look into matter and do needful at your earliest
