Pacl India — not able to select the nominee option while claiming the refund

Hi, i am not able to select the option apart from self option inthe list box. Since the policy holder passed away and i am the nominee of the investment. I am not able to reach out to any of the customer care numbers. Kindly advise how to submit the refund request.
In the docuemnt it mentioned validity mentioned 30 march 2019. I dont have payment receipts. Is it possible to submit the refund request. Really very tough in claiming the amount
