Pancard Clubs — Membership certificate not received, so I can't claim

Dear sir/mam,

I submitted all the original documents at your office while receiving the cheque, but the cheque which I received it had been bounced, that's why I don't have membership certificate any it's copy or any other number also. So I can't claim.
I am attaching the cheque copy for your information, from that please provide me membership certificate copy, so that I can claim and take my money.
