I sunita dhakal have booked a bajaj life policy via booking id[protected] on the basis of phone call given by policy bazaar agent. The policy was subject to clearance of documents and sum assured was 6l and monthly premium was rs. 5000/-. The advance one-month premium amount was debited from my account at the time of booking on 25th oct 2019. However, due to non-avilbality of prescribed documents, i have communicated to policy representative to cancel it and refund my amount. My application no. [protected].
