R Shield Insurance Policy — without informing me they have given r shield insurance policy

Hi my name is bharatsinh raol without informing me they have given r shield insurance and i have received a card a 1 week back but that r shield insurance 1000 amount deducted only.. How it can be possible, and i called so many time for call center they didn't respond properly. Final don't want any insurance policy.. Plz cancel it this policy. I need my money back... I need quick response...
