I mr rajendra gupta residing at 23, michael madhusudhan sarani, khidderpore kolkata – 700023 would like to inform you that bharti axa life agent wrongly opened a new policy which is register with my name whose policy number mention above without my information by misleading me and we are requesting them for cancecllation the policy but they are not taking it seriosly. Mrs payel bhattacharya rm and her senior manager are responsible for all this.

So i kindly request you to please cancel this new policy as soon as possible.
I don’t want to continue this policy for future and take some strict action against them. Otherwise i will take some legal action.
