This is regarding application no 111 dated 13 01 2017 we had submitted the requested copies but still not received the credit for old rs 500 & 1000 notes.
Please let us know urgently when we will receive.

1. Scan copy (Pdf) of all the pages except the blank pages of passport

We have submitted all the documents also emailed.
