Address:Kolkata, West Bengal

I, Ankita Modak, a resident of Agartala, Tripura am writing this to complaint about a serious medical negligence & incompetence happened while treating my mother Ms.Sumitra Modak(Patient) in Apollo Gleneagles Hospital, Kolkata.
Patient Details:-
Name: Mrs.Sumitra Modak
Age:50
W/O: Kalyan Modak
Referred To: Dr.Sudip Roy & Dr.Mahesh Goenka
Bed No: Cabin bed No:496, ICU:268, Super ICU:3517
Admission Date: 21.7.2016
Diagnosis:Gall bladder stone
Elaboration of the incident:
(A)While doing a routine checkup a full body scan was done in Agartala. The scan results were consulted and it was observed that everything was normal except a stone in gall bladder. After discussing this with my father's
company doctor(ONGC) it was finalized to carry on the diagnosis from Apollo Gleneagles hospital, kolkata and she was referred to Dr. Sudip Roy. On 21st July, 2016 my father and my mother arrived at kolkata
and my mother got admitted in Apollo under observation of Dr.Sudip Roy(General Surgeon) . Multiple MRI was done and it was observed that apart from the main stone in gall bladder there were some minute stones exist in the common bile duct.Dr.Sudip Roy then referred us to Dr.Mahesh Goenka (Senior Gastroentrologist)for ERCP because as per doctors those minute stones has to be cleared from the duct first and then the gall bladder stone. We were explained that there was a minor chance of mishap like discomfort in throat, vomiting, loose motion etc but it was never explained that it would lead to death of my mother.
(B) An ERCP was done on 23rd July, 2016 and the day after she got intense abdominal and back pain. Doctor(Mahesh Goenka) initially verbally admitted that while doing ERCP he did something wrong which has turned into
acute pancreatitis and he is trying to treat this. He also told that this will take some time to cure and asked us to hold patience. Pre and Post ERCP result reports were never shown to us, it was just verbally said that the minute
stones were successfully cleared. We did whatever doctors said. They then shifted my mother into ICU. We were also told that we are lucky that she didn't catch any infection and neither there was any internal bleeding.
But after few days we were told that she is now suffering from severe infection and internal bleeding and the infection count is far beyond the normal rate which the doctors couldn't control later. Her whole abdomen
got swollen, her liver, kidney everything stopped functioning. Next day they told us a surgery is required to view the internal organs and to stop the bleeding. surgery was done and they said they had stopped few bleeding points and they kept her abdomen open for 2days for future treatment, but still there was no improvement. After 2days they again took her in O.T and placed a mash around her abdomen and told us that stitch is not required as the mash has been placed
(C) After so many procedures they didn't show us any pre and post surgery reports only we have been informed verbally. Multiple signatures were taken from my father but they never explained for what those signatures were taken when asked they explained those were for medical formalities, we believed them as we were not literate enough in medical science and terms.
(D) Doctors do not discuss the condition of patient adequately with the patient party. specially Dr.Mahesh Goenka who is the senior Medical expert feels very bothered when we used to ask about my mother's detoriating
health condition. He didn't give proper amount of time to treat my mother, everyday he used to visit the ICU for few minutes and used to look around the patients and used to left the situation over assistant doctors.
When my mother used to ask about her own health condition he never replied properly, even he used to give us few seconds to discuss about my mother's condition.
(E) 1 month my mother was in ICU, last 12days before her death she was under ventilation. Still doctors gave us hope that they were trying there best but she was critical. Even me and my family tried to shift my mother to other hospitals but doctors told us why we want to shift the patient as she is going to recover soon.
(F)After 1month of struggle my mother expired on 20.08.2016 at 5:20pm and the death certificate states this as "Multi organ failure due to sepsis with septic shock in a patient of acute pancreatitis with cholelithiasis.
From all organs working to : acute pancreatitis, multiple organ failure and none of them are acceptable and i strongly consider this as a serious case of serious "medical negligence" and "medical incompetence". After
death of my mother, Dr.Mahesh Goenka was nowhere to be seen. Post death medical billing procedure started. And they gave us a bill of 21lakhs along with my mother's body.We were mentally and physically broken at that time. After coming back to my native state Agartala we completed her cremetion and then when we started checking the reports given after her death we found that many prescriptions and main reports were missing. This is the usual scenario in this so called reputed hospital Apollo HOSPITAL and under Doctor like Mahesh Goenka. I would like to obtain the full investigation details. Hope my request would be highly prioritized and an investigation is set up.

I am Rathindra Narayan Bhattacherjee a cardiac patient treating in your Kolkata Unit from 2011. Lastly on 30.01.2016 when Echocrdiography proceedure conducted no Doctor was present except the technician. Although the report was signed by Dr. P.K.TIWARY.
My complaint/enquiry with your Kolkata CEO proved futile. As I am constraint to write to you.
I have admitted my patient to Appolo Kolkta under the guidance of Dr.Mahesh Goenka.Dr.Mahesh Goenka who is the senior Medical expert feels very bothered when we used to ask about my Patient deteriorating
health condition. He didn't give proper amount of time to treat, everyday he used to visit the ICU for few minutes and used to look around the patients and used to left the situation over assistant doctors.
Again there is one Assistant doctor Dr. Aviswanath. He treat us very badly. We have to interact with him couple of times to get the status of patient.Instead of telling the status he scold us everything that why we are asking the status daily. He even don't bothered to see the reports to say the status. He don't have even 1 min to discuss the status. We are going to APollo only with hope of quality medical service and proper care.
